(i.e. windows95/98/ME/XP/NT/2000, Vista, MAC, OSX, RedHat Linux, etc.), the browser
type and version (i.e. Internet Explorer 5.5, Netscape 4.7, Mozilla, etc), the text
of any error messages you are encountering, and a detailed description of the problem.
